who　は直前の　friend　を受ける関係代名詞。
be involved with 〜　＝　〜と付き合う
that　は直前の　group　を受ける関係代名詞。

　take the time, take the courage
that 以下，say の目的となる名詞節で，take 〜 と命令文になっている。

all the kids out there　の内容を説明するのが　who have friends と
２フレーズ下の　and who are worried about them　である。そして
